Scoliosis Screening For 6th and 8th Grade Students

Dear 6th and 8th Grade Parents or Guardians:

To promote health and wellness, we are hosting a scoliosis screening event for all 6th and 8th-grade students. 100% participation is encouraged. If you do not wish for your child to participate, please complete the attached OPT-OUT FORM and return it to Nurse Jackson or your child's teacher before the screening date.

If your child has already been screened by their healthcare provider at their last health office visit, please have the provider complete Form 4400 (ATTACHED) and return it to Nurse Jackson prior to February 11, 2025.

Screening Procedure: The scoliosis screening is a simple process where trained screeners examine the child’s back while standing and in several bending positions. Boys and girls are screened separately.

  • For Girls: Please ensure your child wears a halter, bra, or bathing suit top underneath their clothing on the day of the screening.
  • For Boys: Screening will require shirts to be removed.

If your child is suspected to have a spinal curvature, you will receive a written notification detailing the findings and recommendations for follow-up care.
