Dear TMSA Parents and Guardians,
To ensure the health and safety of your child while at school, it is critical that we have all necessary medications and medical documentation on file. If your child requires medication for conditions such as Asthma, Food Allergies (requiring EPI Pen or Benadryl), Diabetes, Seizure Disorders, or any other medical condition, we need your immediate cooperation.
Action Required:
- Submit Medications and Supplies: Please provide the clinic with your child’s medication, including any required supplies, by September 16, 2024. Medications must be in their original container, not expired, and have a pharmacy label attached.
- Health Care Plan: You must also provide an updated health care plan, dated after June 1, 2024, detailing your child's medical needs.
- Over-the-Counter Medications: TMSA does not administer OTC medications unless provided by you with a completed SHS-1 form. If your child experiences frequent issues such as headaches, allergies, or menstrual cramps, it is advisable to keep an OTC medication at school. Ensure all OTC medications are age-appropriate, unexpired, and in their original container. NO loose pills or opened pill bottles will be accepted.
